Vision Enrollment - Voluntary Plan

Who is Eligible?

• Graduate Assistants with waiver-generating appointments are automatically enrolled in the vision plan once their appointment paperwork is complete. 
  • They may choose to enroll eligible dependents in the plan at their own expense.
     
• Undergraduate and Graduate Students who are not automatically enrolled may voluntarily enroll in the vision plan during the Fall open enrollment period. 
  • Eligible dependents (legal spouse, civil union spouse, domestic partners, and dependent children up to age 26) may also be added at the time of enrollment. 

Enrollment Instructions

• Student Coverage: Complete the Enrollment Form and follow the on-screen instructions. When enrolling in the vision plan, you may also add eligible dependents at that time.

• Dependent Coverage:

  • Undergraduate and Graduate Students: Use the Enrollment Form above.

  • Graduate Assistants: Use the Graduate Dependent Enrollment Form.
